Angel House: A Partner in Care

At Angel House, we’re committed to providing high-quality recuperative care for individuals in need. As a trusted partner to agencies, we offer a safe and supportive environment for clients who require post-hospital care.

Often, patients are released from the hospital with nowhere to go and no one to care for them. Homelessness and inadequate healthcare can create a dangerous barrier to recovery. Without a safe place to heal, illness can worsen, leading to a cycle of suffering. When basic needs aren't met, physical and mental health decline, making it even harder to escape homelessness.

We’re dedicated to delivering exceptional comprehensive programs and services while making a positive impact on the community.

SERVICES

Supportive Services:

  • Case management: Connecting individuals with social workers who can help them access benefits, find housing, and develop long-term goals.

  • Mental health counseling: Individual or group therapy to address trauma, depression, anxiety, or other mental health challenges.

  • Life Skills Development Program: AA-GC, in collaboration with case managers and trusted community partners, offers a comprehensive life skills development program.

  • Transportation Coordination: We understand the importance of attending medical appointments. AA-GC works collaboratively with medical facilities to facilitate transportation for our clients to and from essential healthcare visits.

  • Wound care: Treatment and management of wounds sustained during a period of homelessness or recent illness.

  • Chronic disease management: Support for individuals managing chronic conditions like diabetes, heart disease, or mental health issues.

  • Medication management: Assistance with obtaining and adhering to medication schedules.

  • Pain management: Strategies for managing pain from injuries, surgeries, or chronic conditions.

  • Post-surgical recovery: Support for individuals recovering from surgeries that require a safe and stable environment.

  • Physical therapy: Helping individuals regain strength, mobility, and independence after an injury or illness.

  • Occupational therapy: Assisting individuals in relearning daily living skills or adapting to physical limitations.

Medical and Nursing Services:

Additional Services:

  • Dietary Support: AA-GC provides delicious and nutritious meals tailored to individual dietary needs. Our on-site chefs prepare meals based on specific menus developed in collaboration with qualified nutritionists. We ensure each client receives the dietary support they need for optimal healing.

  • Partnership with Peer support groups: Connecting individuals with others who have similar experiences for encouragement and support.

  • Recreational activities: Offering opportunities for socialization, relaxation, and stress reduction.

  • Spiritual support: Providing access to religious services or spiritual guidance for those who desire it.

Admission Criteria

1.      Must have a primary medical problem and/or custodial care need
2.      Must be physically and psychiatrically stable to receive care in a non-medical respite setting
3.      Must be in need of short-term recuperative care and/or custodial care only
4.      Must be able to perform all activities of daily living (ADLs)
5.      If on Methadone must be in a Methadone Program prior to admission
6.     Must be mentally competent to make decisions for themselves
7.     Must have a primary medical problem that requires support in a non-medical respite setting
 
 EXCLUSION CRITERIA
1.   Scabies
2.   Quadriplegic needing respiratory support
3.   Active communicable infections C. Diff/MRSA/TB / Covid-19
4.   Dementia, memory loss, cognitive impairment
.  Combative, violent behavior
6.   Acute psychosis/psychotic disorders
7.   Unable to make decisions for themselves
8.   Unable to perform all ADLs and requires assistance with transfers
9.  Other detrimental circumstances that would create a hazard to client or others in the Program
I.   No Gang affiliation
II.  No arsonist or intentional fire starter

 2)    BASIC PERSONAL SERVICES:

(a) Continuous observation and supervision, as needed.

(b) Provide client/resident with bathing and personal items needed for personal care.

(c)   Assistance in meeting necessary medical and dental needs.

(d) Assistance, as needed, with taking prescribed medications in accordance with physician's instructions unless prohibited by law or regulations.

(e) Bedside care for minor temporary illnesses.


The daily rate for basic services is $      275.00      or the SSI/SSP established rate, a government-funded rate of $
